Your steps for evaluation make a lot of sense. I like that you train the teacher first, survey, then evaluate. I think we make much more headway as teachers if we get the proper help to get started. Peer evaluation, both at the student and teacher level, makes a lot of sense as well. I think that oftentimes our peers have much more influence on our performance than our superiors. You might also include for the teacher, evaluation by the students. I believe it is very important to know what your audience thinks!
